Headers in mswin32/netinet

I tried to build ncrack on Windows/MSVC. I'm not sure this
is possible due to the highly Posix centric stuff in OpenSSHlib.
Does anybody know? I assume ncrack without OpenSSHlib is no point (?)

Anyway; a stumble block for me was the missing header-guards in
all of the .\mswin32\NETINET\*.h files. BTW. why are these in
upper-case? I'm not sure more than <netinet/ip.h> is needed for
ncrack.

So can you please apply the attached patches?
Should be straightforward.
