It's also quite often the result of a user hitting "report as spam" rather
than "delete" (I'm told they're adjacent buttons.. Bad UI design).

And there's more than a few people out there who intentionally subscribe to
lists, and start hitting the Spam button when they either can't figure out
or can't be bothered to unsubscribe....
