I don't get it. Single mode fiber can easily go 10-20km with low cost optics that do single-strand. You're saying it's rare in the US to have 10-20k households within 10-20km radius from an aggregation place?
Yes, I'm sure there are places where this is the case, but then you aggregate hundreds instead (this works even in sparsely populated areas I'd say) and you're going to have less competition, but you still have the possibility to get competition. In these cases I though agree that L2 backhaul by the fiber operator might make sense.
