makes me wonder if it's only working together with sendmail?

compatible protocol from being used beyond that?

CVP is closed, proprietary and ugly.
Can you teach your favorite MTA to do CVP? I doubt so.
 
On the other side, CVP is a performance bottleneck, especially
for large sites. Maybe milter gives you better performance.
