Microsoft Azure MFA Issues

We use Microsoft Azure MFA on/off campus for staff, off campus for faculty and students at this time.  We have an Azure 
MFA Server on site.  We are getting reports of the questions not popping if the MFA request times out.  Does anyone 
have experience with this and how did you resolve it?
