We meter using netflow, and apply sanctions if they exceed (generous)
statistical limits (3 standard deviations; this avoids renegotiation as
averages rise). Individuals can check their usage at any time, and we
publish generic statistics so that the entire campus community can see the
reasons for the policy.

and I believe they still have similar policies at uwo.ca and utwente.nl.

We ran trials of a couple bandwidth management appliances, but they
performed poorly, and what we have works well.
