I use two of three tools:

rrdtool
        when I may want to play around with the graphs and especially
        if I want to create multiple graphs with the same data set.

gnuplot
        does a decent job, easy to use, quick enough for small data 
        sets and when you have your own aggregating functions already

GD
        using perl and GD you can create very nice looking graphs, I
        have only tinkered with this and other people's code.
