Nope, it happened to me when I configured the router over the network. In my case it was a Cisco 803. Do you know if it uses "cmaker" for both the initial and the enable password? Yep, ive seen this happen aswell, using ConfigMaker with a Cisco 1600. I think it only does it if you configure it over the console port.
