I have not used SpamAssassin, but I do use MailWasher, it is a pretty good program. You mark each email that comes in as spam, and after about a week or so, very few need to be marked anymore and it automagically deletes them for you.

I get about 200 emails a day, 30 of them are legitimate (although that number is higher now since I joined this fine mailling list :).

It's open-source, and there used to be a commerical Windows version by
Deersoft for about $40, prior to their being bought by Network Associates.

Most importantly, in the 9+ months of running it, I've not had it dump any
legitimate message yet.
