Audit report format

Hi all,
After finishing every security audit, I always found it dificult to
categorize various vulnerabilities properly.
And each audit taught me few chapters. 
So, the presentation of report has evolved into pretty professional
format.
The template of report is located at:

I am attaching a template of report. It might help some of us to avoid
reinventing the wheel.

Please suggest modifications.
I hope the evolution will continue.
