I've used BuyVM and Secure Dragon in the US, both been around for awhile and reliable.

In the UK I've used HTTPZoom and OpenITC without issue. Also OVH have some very good deals on dedicated servers. I 
currently have a quad core xeon with 16gb ram and 2x1tb hdds for about £25 a month :)
