ports per IP

When running a scan, is there any way to specify ports per IP? I've already
done a bunch of scans and have a list of open ports per IP and now want to
run scripts on those. I could scan all IPs and use -p and specify all the
open ports but that wouldn't be efficient.

Something like:

1.2.3.4:80,443 2.3.4.5:53,5900
