If those 3 to 4 different locations are each behind routers, would
placing firewalls at each site and creating an encrypted extranet
between them with the web server behind one of the firewalls and
inaccessible to the public be secure enough?

Considering that Cisco's IPSEC tops out at 56 bit encryption, would
that 56 bit crypto provide sufficient protection or is there a higher
size key/algorithm that would be better, perhaps running on the
bastion hosts?

Are all the users at a single location behind a router that would
connect to your site?

        No, i believe there will 3-4 different locations, several
miles apart from each other.
