Disaster Recovery Plan

Greetings!

I am in desperate need of a sample Disaster Recovery Plan. I pledge the highest degree of confidentiality and 
sensitivity. The format I have in mind is a list of potential "disasters," followed by an action list for each. 
However, I am not married to any particular format. Can someone lend me a hand?
