Account Suspension Policy

Hi,
 
The University of Cincinnati is in the process of documenting policy on
a variety of subjects.  I was wondering if anyone has formal policy
documents on Account Suspensions, 2 Factor Authentication and on
removing machines from the network that you might be willing to share?
