Elizabeth,

I would highly recommend against that.  Those two pieces of information are used by many companies to help verify 
information, specifically for the reason that it's easy to remember, but isn't often written down.  Putting it on the 
pledge cards gives extremely valuable information to anyone who happens to run across it.
