actually yes, but it was an online form with an 'early exit' rather than two separate forms. I have some C cgi code to accept the responses and summarize the results. I've put the form back online at this address for you to have a look at: http://infos.panam.edu/survey/ Obviously some of the questions are specific to our University but you can get a rough idea of the thing by clicking Next repeatedly. Some of the paths through the form are dependent on the selections you make. It exits quickly if you claim you have no responsibility for sensitive data. Feel free to take any of the html if you think it is useful, and mail me if you want the C cgi code. regards Graham PS The form as put back online has no access to the directory to save results, so it will print out the final form contents on screen as the final page, so you can see what would have been submitted.
