Not an open source but check out 8e6 url filter http://www.8e6.com/.

 This is a pretty straight forward url filter. I would say simple and
easy. Have been using this for nearly 2 plus years on prodution
environments....no issues ! works great !

application.  The application would need to be AD aware and have transparent
