Freakonomics Radio covered this topic (the voluntary self-exclusion document, not the suit) the other day on my local NPR station. Unfortunately I can't find the podcast. In that particular case, a gambling addict signed an agreement in the state of Missouri to never enter a casino again. He stopped for a while, then found a loophole, then got caught and was arrested for trespassing.
