Us West uses a cisco dsl router that has filtering, NAT and other real router
features.

Why can't the DSL routers ("modems") have this built in? Even
just basic filtering?

I just got a DSL line set up at home, and the Netopia 7100 router I'm
using does support port filtering. I haven't played with it much and I
can't vouch for it in any way, shape, or form, but many DSL providers use
the Netopia box. Now, configuring that filtering looks like it's beyond
any "normal" home user, but the option is there.
