Data Breach Notification Services RFP ?

If anyone has an RFP for data breach notification services (i.e., services performed post-breach) that they'd be 
willing to share, I'd appreciate receiving a copy to peruse, etc..

And, if anyone has contracted for post-breach services only to find out that you wanted/needed certain services and 
they weren't included in the contract, mind letting me know what these "gotchas" were?
