Take a look at Fort Systems Ltd. (http://fsl.com). Their appliance is 
reasonably priced, open source, and one of the most effective appliances I 
have used. 
Of course you could also run their software on your own dedicated hardware - 
or build it from the components at http://mailscanner.info
