for *nix it's pretty easy and there are a couple of ways to do it.
netstat -lp will list the pid and program that's running. Also you
could use lsof... have fun with that. Now, forever and a day ago when I
ran windoze I used this commandline thing called port-audit or
something like that... I was able to find this though,
http://technet.microsoft.com/en-us/sysinternals/bb896644.aspx should do
what you need to do... 

Now this is all *IF* you have access to the system. Otherwise nmap was
mentioned, that's one of the best tools ever made. Other than that
google it.
