I'd also be careful since it is a doctors office about possible HIPPA impact (ie what will you do if they are not in compliance and are disclosing personal/private information?). If they are really wanting this done they should have no problem paying for it. Contact some local firms or professionals and ask to be involved in the process so you can learn from it. Find out if they are really wanting more of an IT security audit or a pen test. Also find out what will be done with the results. Are they open to making any changes to resolve issues found or is this just a test drive for you with using them as a possible reference? I did something like this as a favor, but still got paid for it and the client was a smaller financial institution. Since I had worked in the industry for a few years and had time doing audits/pen tests of my own network and home lab they trusted my knowledge and experience. The OSSTMM (http://www.isecom.org/osstmm/) might also be a good place to start to give you a "checklist" of things to possibly look for. Good luck and make sure you are looking out for yourself.
