I like the idea of providing TTL info with the reason. I'm not sure if it
should be a separate column, but that seems to be appropriate.
This will require accurate documentation, because many users may not have a
concept of TTL, and the name is confusing (Time To Live was originally
intended to be an actual "time" field, not just number of hops). Of course
if the display of the info is gated by the --reason option, we can expect
that someone is willing to do a little reading in order to find the option
itself.

Also, shouldn't -v (or -vv or even higher) imply --reason and maybe
--show-ttl?

I think we should probably not get into the habit of extra CLI options just
for displaying extra information (like --reason does currently).
Overloading -v seems like the best route, but I'd want to see a summary of
the different things that show up at the different -v levels before
choosing a place to put it. My gut says -vv would be a good choice.
