I did not know that they were coming through as comments, but I had
noticed the double periods before. Do you perhaps know of a way to
inhibit this behavior when using the DocBook stylesheets?

I don't. It looks like a bug: the comment should always start on a new
line. If you can't easily fix it, and it is possible and has no other
repercussions, you could start the <indexterm> element on the next
line.
