I think it's a combination of both... they tried back in the day, it broke, and they "parked" it for later.
When Marketing and The World were happy to see that www.insert-favorite-content-url-here.com had AAAA and IPv6 PTR records, who cared whether boring, little-known FQDN's were remembered or not.
And then, all the engineers moved on to some other gig, where they did better because, why not?
