We've been experimenting with Keystone here.  It's the best I've seen, and
that's even counting the higher-cost commercial ones.  It's not perfect but
that's what opensource is for :-).

Keystone seems to be quite the useful package (we use it at NACS also).

Written in PHP. Runs well when compiled as an Apache module.

We use it here with PHP and mySQL.
