Yes, the needed signals should already be present internally in the car processor's and all that should be needed is a software update. There should be no hardware to add or change.

This added DTC and others that can be related to this case (such as "neutral shift engaged while accelerating") should be a mandatory update to all vehicles (not just Toyota), as a way to not only find out what's happening but also to provide accountability to hold the manufacturer (Toyota) to the driver's seat of these incidents.

The secondary question, but not less important, is why Toyota did not implement these diagnostics before? Not to create potentially damning evidence? What other DTCs, for other failure modes (for example, "engine fails to switch off when user presses and holds the OFF button" -- which fault is not an uncommon occurrence in at least one Prius that I know) are also missing? A rather simple logical analysis of the potential faults and their security risks should easily lead to a number of needed DTCs, which must be included, in addition to the ones that are progressively revealed by customer feedback.

Finally, there should be a peer-reviewed, independent mechanism to incorporate and verify customer feedback of faults into the available DTCs. Automated testing in a lab is not enough, as environmental, driving, and other conditions may play a role.
