Updated FW-1 State Table paper

I've updated my paper on the FW-1 state table
(finally got a free moment).  These changes
reflect the new state tables as of FW-1 ver 4.1 SP2.

I would greatly appreciate if some of you guru's
could sanity check the paper.  Also, I still can't
figure out if/how FW-1 does ICMP.  Any enlightenment
appreciated.
