Check out Identity Egine's suite of products.

Also, Bradford Networks has some ability to deal with some of this.

None of these solutions are free.

We are trying to determine a better way to provide guest access.
Currently, because of concerns over being classified as an ISP with
respect to CALEA, our network is “private”, and all access must be
authenticated.  Guests must make contact with a staff or faculty member
with access to a manual system to generate a username / password for the
guest to use to get on the network.  We would like to move towards a
more self-serve model where guests can authenticate themselves.  We know
there are SMS / E-mail systems that would allow a guest with a smart
phone / 3G access the ability to register.  Are there free (for the
Guest) CC systems that just use the card number to authenticate the
user?  What are you using to provide self-serve guest authenticated
guest access to your networks?
