data breach/data loss insurance

Has anyone's institution purchased any cyber-security data breach/data loss insurance?  If so, what information did you 
have to provide the insuring company?  Do you have to provide them updated information quarterly/yearly?  Do you feel 
the insurance has been worth it or do you think it has been a waste of money?  If you have had any breaches, did your 
insurance cover it, or was there some fine print where it wasn't covered?  Any companies you'd recommend?
