Documenting a Red Flags Breach

Looking to find a standard template to use for documenting a red flag
breach.  I am envisioning something that looks almost like a "police
write-up".  Including time of event, impact, background, outcome, etc.  Has
anyone looked into the standardization for internally documenting Red Flag
infractions?  If so would you be willing to me your template structure?
