SACRAMENTO -- The personal security of nearly 50,000 people may have been breached by the California Department of 
Health Care Services.

Social Security numbers were printed on the address labels of letters that were mailed by the department last week.

State employees mistakenly included the numbers in a list of patient addresses. The list was sent to an outside 
contractor, who printed and mailed the envelopes on Feb. 1.

The letters were sent to elderly patients of the Adult Day Health Care program, many of whom are blind or have 
cognitive disabilities. The letters contained notices about changes in benefits.

The state has already begun notifying those who are affected.

State officials have sent them follow-up notices advising them to destroy the envelopes and contact their credit 
agencies.
