datalossdb.org submission: Need more information (Hosting Habitat)

DatalossDB.org recently received a submission about an unconfirmed breach 
that did not have enough details. Initial searches did not find anything, 
hopefully someone on the list knows more. Given the details, we guess that 
the company notified customers but the submitter did not include a copy of 
the notice. If anyone has a copy of the notification letter, URL that 
references the incident or court related information, please contact 
curators () datalossdb org.
