business form for use with encrypted devices

Greetings,

Does anyone have a form that you use to indicate that a given mobile device has been encrypted? I am thinking of a form 
similar to the one we fill out to indicate that there is a justification for using Univ. property (like a laptop) 
off-campus. So, if we take our laptops home to do business, we fill out a form to indicate that there is a business 
reason for such.

The sort of form I am looking for (before I re-invent the wheel and make one) would provide info on the particular 
device (laptop/PDA/smartphone make, model, S/N, etc), the individual it was assigned to, the software the device was 
encrypted with (for example, PGP Whole Disk Encryption, version xxx), and then lastly a place where a witness could 
sign.

Do any of you have a form like that?
