Many of our customers use Secret Server in the environment you described.  Some of the nice features of a centralized 
web-based password manager:
* audit password usage
* share common passwords (corporate credit card for marketing staff, server room combination for admins, certificate 
signing password etc)
* Full encryption in a central database with no risk of all exports leaving the company

It is inexpensive ($389 for 10 users) and actively developed and supported (3.0 is coming out in June 2007).
