risk management questionnaire

Does anybody have a link to a free risk assessment questionnaire? I want
to ask regular IT person (not security specialists) questions like "do
you use unencrypted connections (like regular ftp) to external partners?
if yes rate the risk for the business depending upon the data transfered
with low, medium or high." It would be nice not to creat it myself but
download a first version somewhere and then configure it for my
organization.
Any help would be appreciated.
