duplicate key entry error

getting errors like the following once in a while

 Database ERROR:Database ERROR:Duplicate entry '1-14687' for key 1

any idea what causes this?  its only once in a while but it sounds
troubling.. possibly a row locking issue with an insert ?

any help appreciated.
