If you're doing it internally / white box, it's easy: ask the switches and routers which MACs and IPs they've seen lately, what ports they're on, and whether or not the ports have a link. Feed that into your VA tool. If you have an inventory management tool, you know it'll be wrong. Use the above to correct the tool. If you're doing it grey/black box, others have already answered.
