Looks like akamai is going default ipv4 and ipv6 for new customers. https://blogs.akamai.com/2016/06/four-years-since-world-ipv6-launch-entering-the-mainstream.html AWS / Cloudfront / Fastly - please have a look at how it is done. I think Cloudflare already did this.
