This provision was requested by the large insurance carriers, who want to be absolutely sure that they have your complete medical history. That way, they can be sure to refuse to insure you if you seem a bad risk, or find new reasons to deny coverage of a problem if anything in your medical history suggests that it might be "pre-existing."
