Finding account lockouts

Hello All,

I am polling the group to find out what tools each of you use to find the source of account lockouts.  When someone 
calls the help desk and says "My account keeps locking" what are you using to determine which machine has bad cached 
credentials, which machine they are using the wrong password on, what web application they are using the wrong 
credentials on, etc.

I have been using Microsoft's Account Lockout Tools for a while but sometimes they don't fully help.
