Today's interesting observation: I've seen the same "replied to the wrong thing" 6 times just today. In all 6 cases, the people were top-posting their replies. I almost never see this sort of miscue when people bottom-post their replies (probably because you then tend to actually *look* at what you're replying to). Conclusion: MUAs that by default top-post are a security issue, because they make it much more likely that somebody will miscue and release sensitive information.
