Form stating a stolen/lost laptop held no sensitive data?

I was wondering if anyone out there has a standard form an employee or student can fill out when a portable computing device (PDA, laptop, etc.) supposedly containing no sensitive data is lost or stolen? What are you doing to protect the university from liability if the user insists that no sensitive information was stored locally on that machine?
