Code Signing Certificates

We are now getting requests from individuals for University-issued
digital certificates for code signing.  To ensure that everyone involved
understands their responsibilities, we would like to draft a basic
document that describes these responsibilities and defines some basic
expectations.

Does anyone on this list have a policy and/or procedure for issuing code
signing certificates or know of a place that might have such information?
