Our University uses Ethics point which provides a telephone hotline and
a web based system to report any incidents or areas of concern.  What I
like most about this system is it provides a means of follow-up.  The
person who receives your report has the ability to ask follow-up
questions, or even schedule a real-time chat.  More information is
