Incident Reporting

A few questions on campus security incident reporting.

Reporting to outside agencies (external to your institution)
1) What criteria does your organization use to evaluate when to report an
incident?

2) Who makes the determination as to report or not?

3) Who (what organizations) do you report the incident to?  Please list in
order depending upon severity.
