datalossdb.org submission: Need more information (Fuddruckers)

DatalossDB.org recently received an anonymous submission that did not have 
enough details. Initial searches did not find anything, hopefully someone 
on the list knows more. Given the "Org mails notification" date, it 
suggests this may be a distinct event and not related to the more recent 
payment processor breaches (e.g., RBS, Heartland). If anyone has a copy of 
the notification letter or a URL that references the incident, please 
contact curators () datalossdb org.
