utmp entry is wriitten by xterm -- not by shell. It is controlled by xterm's options: +ut and -ut Login shell is controlled by xterm's options: +ls ans -ls
Login shells make a note on the log file to indicate you were there. Non- login shells (or subshells) on the other hand don't.
No -- it is done by programs what starts login shells....
